The last set of molars to develop is the Wisdom Teeth (or Third Molars), which usually begin to develop between the ages of 17 and 25. Extraction of such a tooth is a common surgical procedure recommended in instances where it is painful, infected, or crowded, or when it is trapped (impacted) and cannot fully erupt into the dental arch.
Essential Characteristics and Removal Reasons
- Diagnosis & Assessment : Treatment commences with a comprehensive clinical examination and Radiographic Imaging (X-rays/OPG) to determine the location, angulation, and stage of root formation of the wisdom tooth, as well as its relationship to other vital structures, such as nerves and sinuses.
- Affected Teeth : Impaction, which is an obstruction of the tooth by the jawbone or by another tooth, is the most common cause of removal. This can lead to:
- Pericoronitis : Tooth infection and gum inflammation of the gum tissue around a partially erupted tooth.
- Crowding/Misalignment : Tension on neighbouring teeth can lead to all the arches being out of alignment.
- Cysts or Tumours : In rare cases, it may also lead to the development of cysts or benign jawbone tumours in the affected teeth.
- Procedure : The removal procedure is conducted under local anaesthesia (or other sedation, as necessary). It begins with a simple extraction of fully erupted teeth and progresses to minor surgery for impacted teeth, where small incisions are made to access the tooth. In other cases, the removal of overlying bone is performed gently.
Indications for Removal
- Pain and Infection : Acute or chronic events of pericoronitis.
- Dental Caries (Decay) : When the tooth becomes too hard to clean, decay of the wisdom tooth or the neighbouring second molar can occur.
- Orthodontic Necessity : Removal to provide the space required or avert relapse of orthodontic correction.
- Pathology : Cysts, tumours, or jawbone damage associated with the tooth are present.
